I know that sounds really noobish of me, but I've always just tightened to the point that it's tight, although I don't think I want to try such a procedure with the transmission.
1) Yes. You have to hold the adjuster still while loostening the lock nut.
2) You said that as you understand it, you have to tighten it. Turn the adjuster clockwise.
3)Years of experience, or remove the transmission so that you can measure it.
