Sometimes it's just one bad heat spike that ruins a tranny for good. It won't start sliding on the spot, but the damage is done to the clutches. Or maybe the fluid is just burned and it was left in there. Maybe there was a restriction that dropped the hydraulic pressure, causing the clutches to slip more that they should.

I could go on saying 'maybe' all day long because the conditions which cause the damage could come about any number of ways. The fact that it happens across such a wide number of miles on our cars is just down to statistics.
