To most people I reccommend only the three piece set.
Less work, but less temp protection.

Are the throttle body gaskets attached to the throttle body spacer? I don't have any independent gaskets in my kit...

If you use the five piece set, they go between the heads and the stacks.
But you have to grind the webbing on the top manifold to clear the AC line.

The gaskets should be included in the kit.
If they were attached they would be green.
