Front Stock speakers are spot-drilled to mounts. Get a drill bit big enough to go through the middle of the spot drills. When you've drilled through them all, pry old speakers from the housing. Attach new speakers to the housing (you may need to drill 1 or 2 new holes to accomodate mounting screws.
It's not as complicated as it seems. It took me 15 min per speaker/mount to do the swap!

I'd suggest making a baffle/bracket to hold the speakers. I made mine out of MDF, and it has worked well for over a year now. just use the stock speaker bracket as a guide. I'd suggest using a 1/2 inch or smaller wood. (you're going to want to seal it so that it doesn't rot)
As for the rears, you have to take out most of the rear interior to be able to get at them from the top. Search in the 'how tos' for how to do this.

My speakers were too small for the hole as well, but there was just enough room to center the speaker mounting holes.....AFTER I bent the roll-over diameter on the door panel. It worked out okay....I put some extra insulation padding around the edges as well. I've got a pic or two of my speaker in my locker.

Reading
The more I read the better my stock speakers sound. Oh well, doesn't make much difference since I don't hear to well anyway.

You should consult the SVXipedia on http://svx-iw.com/
There's 2 how-to's on replacing the speakers there.
