Throttle cable

I've recently experienced some hesitating when I start from a stop. As I accelerate, the rpms kick up to around 1500 and then drop to 1000, causing the car to surge, then it will accelerate at normal again....all while keeping my foot on the gas.
I did the throttle cable adjustment a year and a half ago when I first bought the car to eliminate the hesitant starts, and that did work.
Do the cables need to be adjusted again? Is this a normal mod that needs to be checked every now and then. I can't imagine I stretched the cable....

Cable actually had a bit of slack in it.....So I tightened it up...so far symptoms are gone.....
