Only a small detail mate.
If this doesn't happen when the engine is warm, it has to be a lean mixture. When you start the car it increases the air/fuel ratio by doing double injections, each injector fires every rev, instead of once every second rev. This is when the highest fuel flow is needed, as I believe it is lean, there might be a flow restriction. Could even be the pump running on the resistor all the time to reduce its output.
Still can't tell you what it is, may be some body has some experience of a flow problem before.
