Grab a wire coat hanger, bend a short 90 degree bend at one end and trim it fairly short, about a 1/4" long or so. Stick it in the spark plug hole and gauge the depth you need thean add enough to that to curl around for a grip. Cut, curl and stick it down the hole and get the dang boot outta there. Heck, I fish these things outta all kinds of cars, Toyotas, Datsuns, Neons, etc., etc.

For the replacement boots stop at a part store and get the cheapest set of wires they have with straight dist. boots. Or stop at a tune-up shop and rob some from the dumpster. Or again, stop by a Toyota or Datsun dealer and grab some from an old set of thier's (actually their spark plug ends are almost identical to the boots you're looking for due the depth of their wires.). I know I've tossed 15-20 sets in the past six months. I'll try to make a habit of saving them in the future.
