I'm not sure what a Flat rate manual is. Looks like it's actually two parts catalogs in one big binder. First manual is 92,93,94 CAT-NO.EAU1-05. The second is 94,95 CAT.-NO.US-C11-003. One other thing I should mention that I just saw was someone cut the bottom right corner section(about 2.5"x2.5")out of the last two pages and third/fourth pages to last are just cut, but are all there. Hope this helps.
