The timing belt slipped. The timing belt shield broke and a chunk fell into the pulley and allowed the belt to slip.
I can't figure out why it didn't throw a code. Shouldn't it if the cam angle sensors don't match readings??

Well thats weird Doug, What broke the cover?
The cam position sensor was working ok, it was just telling the ECU that the wrong set of three cylinders were ready to fire.
