Here in Quebec Canada if the car is resold in Quebec there is no inspection and no issue at all with modifications. A recent law makes it impossible to license a right hand drive car in Quebec but not the rest of Canada. Existing owners of right hand drive cars are exempt.

If a car is imported from another province or country it must pass a basic safety inspection. Modifications are allowed as long as it does not affect the safety of the vehicle.

Additionally any car imported from another country into Canada must meet Canadian regulations for safety, crash tests etc. and may require modifications to meet these standards. Things such as bumpers, speedometer and lighting are common ones needing to be changed.
