I remember that you have to bend the plate into a bow shape to get it to fit flush with the bolt holes. Then, if I recall correctly, that you have to line up the holes in the 'wings' with the factory frame holes and then relocated driveline bearing gets bolted to the other holes(forward) in the plate.
