It only applies to XP when you use IE7 and Outlook 2003 for your Email.
The issue is that every time you open an HTML formatted message or try to compose one, the mail system will scan the entire list of Internet Explorer restricted sites at every click or keystroke. the registry setting that winmail.reg changes is to set the mail program to not scan the restricted sites list every time you do something.
The problem started May 9th with a Microsoft update and as users install the updates, the problems are surfacing.
MS contends that the issue is not something they care to resolve as they never intended the restricted sites system to actually hold all of the restricted sites out there.
