O/T: Useful form style response letter
 

I realize this is perhaps deserving of me receiving what I am offering to the forum (is that circular enough?); however, the file link below points to a form letter used to respond to certain types of posts, letters, etc. Maybe some of you will find a use.

Then there is the complaint form:

Write you complaint in the box below. Please write clearly and stay within the lines of the box.

Along with the completing the complaint form, you need to submit it. This is done in an orderly fashion. I've seen two methods. The first is "Take a number". This number is attached to the pin of a hand grenade.

The second is to "Press the button for service". This button is mearly a smiley face attached to the business end of a mousetrap.
